Some areas (your bedroom, the hospital, the Entropic Woods room where you begin Act 2, and the Entropic Sky area where you begin Act 3), autosave the player when you visit them for the first time. There was a bug with the autosave that resulted in the player accidentally being saved into the room you were previously. If you close out of the game, then load back into a room that has autosave, this could result in the player file setting the Title screen as your last save position (essentially making your save file unplayable).
